Requirements

Independent
- but help needed to maintain personal safety and adequate health
status (e.g. proper diet, controlled medication, assistance with
housekeeping and laundry, etc.)

Applicant
has minimal physical and/or mental frailty but is not confused to
the point where safety of self or others would be endangered.

May
require minimal personal care (e.g. with baths, etc.) requires little
or no professional nursing and medical care.

Would
benefit from socialization and recreation programs.

May
require aids such as handrails, grab-bars, walker, etc. to assist
with activities of daily living.
-
Is
able to ambulate by self at least one city block (with or without
mechanical aids).
